Notice of Privacy Practices 

  • Personal Protected Information (PPI) is any information Loaves and Fishes has about a participant that allows for the identification of that individual.
  • Loaves and Fishes will only collect and/or disclose PPI for the purposes stated in this notice.
  • Loaves and Fishes will alert you prior to any disclosure not stated in this notice.
  • Loaves and Fishes will maintain the confidentiality and security of all PPI collected.
  • You may request to access, inspect and/or correct PPI related to the services you receive from Loaves and Fishes that is maintained in your resident file.
  • You may challenge, through established grievance procedures any denial to access, inspect and/or correct PPI related to the services you receive from Loaves and Fishes that is maintained in your resident file.
  • Loaves and Fishes will make every effort to maintain complete and accurate PPI on residents served and collect that PPI in a timely manner and dispose of that information in a confidential way that complies with this notice.

How and Why Loaves and Fishes Collects PPI

Loaves and Fishes collects PPI only when appropriate to provide services or for another specific purpose of our organization or when required by law. Loaves and Fishes may collect information for these purposes:

  • To provide individual case management,
  • To produce aggregate-level reports regarding use of services,
  • To track individual program-level outcomes,
  • To identify unfilled service needs and plan for the provision of new services,
  • To allocate resources among agencies engaged in the provision of services,
  • To operate Loaves and Fishes including administrative functions such as legal issues, audits, personnel planning, program planning, evaluate and oversight and management functions,
  • To comply with government reporting obligations for Homeless Information Systems.

Loaves and Fishes only uses lawful and fair means to collect PPI. Loaves and Fishes normally collects personal information with the knowledge and consent of our participants. During the Intake Interview, a staff member will describe the policy and practices within this notice and collect the participant’s written consent for release of PPI for the purposes listed above.

Loaves and Fishes posts a sign at our intake station explaining the reasons we ask for personal information.

Donor Policy

Loaves & Fishes of STL values and respects its donors’ right to privacy. It affirms that its fund-raising practices protect against unauthorized use of its contributor lists, and permits no general telephone solicitation of the public. Loaves & Fishes of STL utilizes donors’ personal information such as names, mailing addresses and email addresses only for purposes of donor acknowledgement and/or when an individual voluntarily provides this information and opts to join our mailing list. Loaves & Fishes of STL does not share donors’ personal or contact information with any third party organizations. Visitors to the Loaves & Fishes of STL Web site remain anonymous. Loaves & Fishes of STL does not require visitors to register or provide personal information to view the site. There are instances when site visitors are asked for additional information in order to provide requested services. For example, information will be requested when making an online donation to Loaves & Fishes of STL so that the gift may be properly processed and receipted. Online donors choose the personal information that Loaves & Fishes of STL receives and can opt to remain anonymous. All credit card transactions are conducted over a secure server.

Loaves & Fishes of STL uses a number of web analytics products to optimize our website, including Google Analytics and Google Website Optimizer. These tools do not require our site visitors to identify themselves, and they do not provide us any personally identifiable information about our visitors that is not supplied voluntarily. Our goal in using analytics is to understand what parts of our website you like and which parts need work.
